Home Health Physical Therapist - Part-Time - Make a Difference with Optimal!

Optimal Home Health is seeking a†PT to cover the Visalia area

JOB SUMMARY:

The physical therapist contracted or employed through the organization is responsible for providing physical therapy services. Ability to demonstrate the knowledge and skills necessary to provide care, based on physical, psychological, educational, safety and related criteria, appropriate to the age of the patients served in his/her assigned service area. Have knowledge in anatomy, physiology, body mechanics, assistive devices, gait training, range of motion and other therapeutic treatment modalities. Knowledge of third party payers as well as accrediting body, local, state and federal regulations. The skills and knowledge needed to provide such care may be gained through education, training or experience.

QUALIFICATIONS:

  1. Possesses a degree from a baccalaureate or masterís program in physical therapy approved by an accredited organization.
  2. Is licensed to practice as a physical therapist within California.
  3. Possesses and maintains current CPR certification.
  4. Must be able to left, bend, climb stairs, and negotiate difficult pathways in and about patientís homes. Work in confined areas, support patients in standing, sitting, transferring and walking. Lift and support patientís extremities while giving care, and turn patients. Move furniture and/or carry light objects if indicated. See, hear, and speak without limitations for accurate verbal and written communication.

JOB DUTIES:

  1. *Provides physical therapy services to patients according to a written physicianís plan of care. This may include, but is not limited to:
  1. Assessing and evaluating therapeutic/rehabilitative/functional status, and participating in the development of the total plan of care (revising it as necessary), and at least every 30 days the qualified therapist functionally reassesses the patient for ongoing therapy services.
  2. Assesses for muscle strength, mobility, gait, range of motion, balance, safety and transfer capabilities.
  3. Instructing patients/families/caregivers in the use and care of assistive devices.
  4. Determining priority needs for physical therapy and identify resources to address patient population needs.
  5. Reporting to physician patientís reaction to treatment or changes in condition.
  1. *Participates in teaching the client and family. Instructs, motivates, and assists patients to learn and improve functional activities such as transfers, ambulation, and daily living activities.
  2. *Fits patients for supportive devices such as crutches, canes, walkers, and wheelchairs.
  3. *Measures patientsí range of motion, length, and girth of body parts and vital signs to determine effects of specific treatments in the clinical record.
  4. *Initiates physical therapy program and instructs other personnel and/or family/caregivers in certain phases of physical therapy program for the patient by participating in case conference.
  5. *Educates patients in the use of prosthetic devices.
  6. *Supervises certified home health aides when such team members are participating in patientís plan of care and nursing is not involved at the time.
  7. *Prepares and submits clinical and progress notes based on the attainment of goals/outcomes. Plans/implements patients discharge appropriately.
  8. *Provides physical therapy consultation to families/caregivers, as appropriate.
  9. Participates in and may provide in-service education programs.
  10. *Participates in the Interdisciplinary team.
  11. Participates in the quality management program.
  12. Supervises paraprofessionals as necessary.
  13. Maintains agency defined productivity.
